Do DSM-5 substance use disorder criteria differ by user care settings? An item response theory analysis approach

چکیده
To examine differences in the psychometric characteristics of diagnostic criteria for Substance Use Disorders (SUD) between substance users harm reduction settings (HR) and seeking treatment (Tx). Differential Item Test Functioning (DIF & DTF) analysis were performed to difficulty endorsement discrimination 11 test if set as a whole (the “test”) functioned differently by care (Tx vs. HR) alcohol, cocaine, cannabis, opiates tobacco. uniform nonuniform DIF, multiple indicator cause (MIMIC) structural equation models used. Regardless substance, DSM-5 “craving”, “large amount”, “time spent”, “tolerance” “activities given up” had similar functioning settings. Little evidence DIF was found other criteria. While majority related loss control use, well both settings, consequences use several differential functioning.

The Puzzling Unidimensionality of DSM-5 Substance Use Disorder Diagnoses
There is a perennial expert debate about the criteria to be included or excluded for the DSM diagnoses of substance use dependence. Yet analysts routinely report evidence for the unidimensionality of the resulting checklist. If in fact the checklist is unidimensional, the experts are wrong that the criteria are distinct, so either the experts are mistaken or the reported unidimensionality is sp...
متن کاملVariability in the prevalence of adult ADHD in treatment seeking substance use disorder patients: Results from an international multi-center study exploring DSM-IV and DSM-5 criteria☆☆
BACKGROUND Available studies vary in their estimated prevalence of attention deficit/hyperactivity disorder (ADHD) in substance use disorder (SUD) patients, ranging from 2 to 83%. A better understanding of the possible reasons for this variability and the effect of the change from DSM-IV to DSM-5 is needed. METHODS A two stage international multi-center, cross-sectional study in 10 countries,...
متن کاملAn item response theory analysis of the DSM-IV borderline personality disorder criteria in a population-based sample of 11- to 12-year-old children.
Although a growing body of empirical literature provides some support for the diagnosis of borderline personality disorder (BPD) in youth, little is known about the internal structure of BPD and the performance of the individual diagnostic criteria, especially in younger samples. We used item response theory (IRT) methods to investigate the psychometric properties of the Diagnostic and Statisti...
متن کاملDo DSM-5 Eating Disorder Criteria Overpathologize Normative Eating Patterns among Individuals with Obesity?
BACKGROUND DSM-5 revisions have been criticized in the popular press for overpathologizing normative eating patterns-particularly among individuals with obesity. To evaluate the evidence for this and other DSM-5 critiques, we compared the point prevalence and interrater reliability of DSM-IV versus DSM-5 eating disorders (EDs) among adults seeking weight-loss treatment. METHOD Clinicians (n =...
